Description PRIMARY FUNCTION: Primary support and case manager for
individuals with mental illness by assisting them to live as
independently as possible in their chosen community. In addition,
this position provides knowledge of a specialty area to the
treatment team and program. REPORTS TO: Clinical Director
SUPERVISES: No Direct Reports for this position CLASSIFICATION:
Full- Time SALARY RANGE: $17.61 - $21/hr (Negotiable Based on
Experience) Divine Changing Hands, LLC (DCH) is a culturally
responsive sobriety focused organization that serves the whole
client. Our intensive outpatient treatment facility focuses on
creating a safe environment where our clients receive supportive
mental, physical, and spiritual health services. We offer hands-on
guidance to our clients through our licensed clinical staff,
trained behavioral health technicians, and certified peer support
specialists. Our services are offered to enhance client success in
four domains: mental strength and stability, physical wellness,
safe environments, and culturally responsive programming. DCH is a

PRIMARY RESPONSIBILITIES: Reporting directly to the Chief Clinical
Administrator, the Case Manager is accountable for resource
identification and connection that enhances participant success.
The Case Manager is responsible for collaborating across functions
to drive org-wide connection to resources and best practices that
create long-term participant success. They will lead resource
allocation and identification that aligns with holistic behavioral,
mental, spiritual, and physical wellness of all participants. Under
general direction, the Case Manager will provide case management
along with service and resource coordination in a human services
environment, particularly with participants that are traditionally
underserved and in crisis or emergency situations. ADMINISTRATIVE
FUNCTIONS: Manages documentation using EMR for treatment progress
and organizational purposes. Serves as a vital member of a
multidisciplinary treatment team to assist participants to reach
goals of self sufficiency. Attends community meetings that pertain
to resources specific to services. CLINICAL SERVICES: Conducts
intake assessments including genograms and ecomapping to complete
individualized case management plans. Collaborate with members
served to create Recovery Plan goals and ensure all services
described in the Recovery Plan are provided Counsels participants
to facilitate achieving individualized plan goals, developing life
skills, and/or sustaining financial stability, conducts activities
and monitors the service environment in a manner to maximize
participant success and well-being. Consistent with area of
expertise, provides specialized services and interventions related
to specialty, including thematic groups, evidence-based practices,
and collaboration with external agencies and resources Facilitate
groups according to the needs and interests of members served in
the area of specialty Assist members served with all needs,
including Activities of Daily Living Assist members served in
making and getting to essential appointments Assist and advocates
for members served in obtaining appropriate benefits; provide
representative payee services Safeguard confidentiality; adheres to
HIPPA policies Provide wraparound services that encompass whole
person care Provide transportation when needed and appropriate and
linkage to community resources RESOURCE DEVELOPMENT Maintain
awareness of community resources and assists members served in
appropriate use of community resources Works with community
partners to maximize the assistance for participants. Advocates on
behalf of participants for services, basic needs, and other related
issues. ESSENTIAL SKILLS AND FUNCTIONS: Reliable work attendance,
ability to respond to after-hours emergency situations. Astute
problem solving and communication skills with a diverse audience
Exceptional interpersonal skills with extensive demonstrated
experience in collaboration across multiple teams and delivery
functions Demonstrated experience in community resource
identification and establishing new community partner relationships
to enhance participant long-term success. Experience documenting
participant progress using DAP/SOAP notes in EMR A professional and
resourceful style with the ability to work independently and as a
team player, to take initiative, and manage multiple tasks and
projects at a time Ability to plan, introduce, and lead a process
that enables high-quality growth A passion for working with
vulnerable populations and a strong commitment to individual
success Commitment to diversity and inclusion Ability to
communicate effectively orally and in writing. Demonstrated ability
to meet deadlines and carry out responsibilities with minimal
direction. Work cooperatively with other agency staff and community
and state agency representatives. Familiar with Microsoft Word and
comfortable in a Windows/Microsoft Office environment. MINIMUM
QUALIFICATIONS: Four (4) years' experience in a behavioral health
setting Experience providing intensive case management and/or
intensive community supports using psychiatric rehabilitation
concepts Experience in a mental health setting, outpatient clinic,
dual diagnosis treatment, or community mental health service
Experience in and knowledge of a specialty area to be provided with
general case management of the team. Maintain a valid Arizona State
Driver---s license and have a personal vehicle with insurance.

equipment; talk and hear; stand and walk, reach with hands and
arms; stoop or kneel. Specific vision abilities required by this
job include close, distance and color vision and the ability to
