responsibility of ensuring an exceptional end to end patient
experience, which includes directing patient care through the
continuum in a fast-paced outpatient environment. Our treatment
philosophy is a promise to provide an engaging experience in mind,

principles of Applied Functional Science (AFS). The therapist who

for the position. This position reports to the facility clinic
director.Essential Job DutiesEvaluate patients using appropriate
tests, measurements, clinical observation and supplementary
information provided by the patient.Develop individual patient
treatment goals and activities based on evaluation of patient data
and demographics.Administer treatment to improve and maintain
function and evaluates its effects.Instruct, motivate, educate, and
assist patients to perform various physical activities designed in
the treatment plan.Monitor effects of treatment at various stages
and adjusts treatment to achieve maximum results.Document
treatment, response, and progress results onto chart and
transcribes notes as required.Communicate with medical
practitioners to obtain additional information, suggest treatment
revisions, and integrate therapy alternatives into patient's
care.Provide direction and communicates with the physical therapy
technician appropriately for assistance in completion of patient
care.Create an educational environment for the technician within
which to learn.Maintain patient records in accordance with State
and Federal guidelines, HIPAA compliance, and company
policies.Assist and participate in the marketing of programs,
services, the facility, and the organization as a whole.Meet all
continuing education, licensing, and compliance requirements as set
forth by the APTA and Spooner guidelines.Take proactive measures to
follow and remain current on professional documentation and
reimbursement guidelines.Participate in clinic in-services and
staff meetings to enhance the learning environment.Education and
ExperienceGraduate of APTA accredited physical therapy
institution.Licensed in the state of Arizona.CPR
