Job Description:
We are looking for an FNP or PA experienced in Family Practice
that would like to work with a group of individuals that are
patient oriented and easy to work with. We believe in Family first
in private life as well as our patients life's. We will need you to
able to, Coordinate care with medical team members to ensure
patient receives comprehensive healthcare. Asses patient needs,
Annual wellness visits, evaluate current treatments, make
adjustments if needed to ensure positive patient outcome. Acute
care as well as Chronic disease management.
