MD or DO degree Board qualified or certified. AZ licensed physician credentialed SJHMC membership in appropriate professional associations and societies and successful completion of residency program required Job Requirements Successful completion of residency program preferred. Experience with a craniofacial/ maxillofacial population preferred. Credentialed at St. Joseph?s. ACPA involvement preferred.
