PM Experience: Minimum of 2 years social work or case management
experience in an inpatient setting preferred. License: Current
Registered Nurse or Social Work licensure. Certifications:
Certification in Case Management or Rehabilitation preferred.
Must-Have Skills/Experience: Basic computer knowledge. Effective
communication skills. Ability to communicate effectively in English
(verbal and written). Description: This position is for an RN Case
Manager specializing in case management within an inpatient acute
rehabilitation setting. The role focuses on the treatment and
recovery of individuals who have experienced the severe effects of
injury or illness. The ideal candidate will provide ongoing care
and specialized treatment tailored to the specific needs of
patients recovering from various conditions. Responsibilities
include maximizing health, function, and quality of life through
comprehensive physical medicine and rehabilitation programs.
Patient population includes individuals recovering from stroke,
brain injury, neurological conditions, trauma, spinal cord injury,
amputation, and orthopedic injury. Experience in inpatient
rehabilitation is strongly preferred. The work environment consists
of a 40-bed unit with a patient ratio of 1:13. The electronic
health record system used is Cerner. Candidates should have a
