(with select medical plans)Free Short-Term Disability and Life/AD Insurance up to $100,000403(b) retirement plans with company match.Employee Assistance Program Voluntary benefits: Long-Term Disability, Pet Insurance, Additional Life/AD Insurance, and much more!Who We NeedWe're looking for someone with the following skills, experience and credentials:Carries a short-term caseload of up to 20 comprised of individuals who are frequent utilizers of inpatient and emergency department settings.Serves as an added resource to the assigned primary case manager to facilitate individual engagement and follow-up in care, understand and implement post-discharge/post-visit medical orders and to coordinate more specialized post-discharge/post-visit placements (including medical specialists, skilled nursing centers, home health, and residential settings).Coordinates with the Care Management Staff in identifying potential causes of high utilization and assists the Integrated Care Team in designing and modifying interventions to address the issues identified.Monitors members (in person and by telephone) for changes in clinical symptoms and treatment side effects or complications and communicates back to the team.Supports medication management prescribed by the BHMP or PCP, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment within their scope of practicTrack member follow-up, gaps in care and clinical outcomes using the NextGen EHR and other population health toolProvides support and education to families and natural supports on treatment options.As appropriate to license, training and/or clinical oversight, provides brief interventions using evidence-based techniques such as behavioral activation, problem-solving treatment and motivational interviewing.May also utilize standard screening tools for physical and behavioral health conditions. Other related duties as assigned.We have a full-time position that works Monday-Friday, 8AM-5PM. Location is 4330 E. University Dr. Mesa, AZ 85205.Job requirementsMaster's degree or Bachelor's degree in a behavioral health related field Bachelor's degree.Minimum of one (1) year of work experience in medical social work and/or behavioral health case management in an integrated setting required.If High School High diploma or high school equivalency diploma is highest education, a minimum of four years of full-time behavioral health work experience is required.Ability to obtain and maintain a Level One Fingerprint Clearance Card.Valid AZ Driver's License.Successfully complete a pre-employment drug screening.Must be able to lift minimum 50 pounds, stoop and bend comfortably in all positions.Copa Health is an Equal Opportunity Employer - All qualified applicants will receive consideration for employment without regard to race, color, religion, national origin, disability status, protected veteran status, or any other characteristics protected by law.
