Job DescriptionPatient Care CoordinatorAegis Health Care is hiring Healthcare Heroes!Aegis Health Care is currently seeking a detail-oriented Patient Care Coordinator to be part of our Atlas Medical team. Atlas Medical specializes in providing primary care for geriatric patients in Senior Living communities. This is a full-time position working Monday-Friday 8am-5pm in our Mesa, AZ office.The Patient Care Coordinator (PCC) is an important part of the Aegis Healthcare team. Aegis Health Care specializes in providing complete care for geriatric patients. The PCC provides planning, direction, and coordination of overall healthcare of residents in senior living communities. You will collaborate care between the facility, family, and providers that serve the patient.Atlas Medical is part of Aegis Health Care. Aegis is unlike any other health care company.
